Position Overview: We are seeking a detail-oriented and empathetic Medical Assistant to join our remote care team. In this role, you will support our licensed clinical staff by assisting patients with outreach, monitoring, and coordination. You’ll play a key role in ensuring patients feel supported and no care needs go unnoticed.

Key Responsibilities

Conduct patient outreach calls for Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs.
Perform medication reconciliations and help patients understand care plans.
Support patients with device setup, troubleshooting, and non-clinical questions.
Monitor biometric device data, flag abnormalities, and escalate clinical concerns to nurses.
Document interactions, update patient records, and ensure accuracy in EMR/EHR systems.
Assist with administrative tasks such as preparing reports and uploading documentation.
Collaborate with team members in daily workflows, huddles, and ongoing training.

Qualifications

Medical Assistant credential
Minimum 1 year of experience in a clinical, telehealth, or care coordination role.
Proficiency with EMR systems, digital tools, and comfort working in a remote environment.
Strong communication skills, with the ability to build rapport over the phone.
Highly organized with excellent attention to detail and follow-through.
Team-oriented, proactive, and committed to patient-first care.

Preferred Experience

Prior experience with CCM, RPM, or BHI programs.
Experience supporting patients in a telehealth or remote care delivery setting.
